Performs resource management,facilitation, and referral to other levels of care. Workscollaboratively with the multidisciplinary care team to facilitateachievement of desired treatment outcomes. The social workerintervenes with patients who have complex psychosocial needs,require assistance with eligibility determination for socialprograms and funding sources, and qualify for community assistancefrom a variety of special funds and agencies. The social workersserve as patient advocates and counselors, perform psychologicalassessments, refer patients and families to medical resources andprovide patient and family assistance in obtaining financial andlegal assistance. In addition, offers crisis intervention topatients and families with psychosocial needs and collaborates withthe patient care team in the development of care for all patientsincluding high-risk patient population. The social worker will alsoparticipate in ongoing management of these patients. Mostimportantly, a LICSW/LCSW works to assure that the best interestsof the patient are met.Provide healthcare services consistent with state law and therequirements of the professional licensing or certificationauthority and/or hospital privileges, where applicable. Suchservice may include, but are not limited to.Minimum Qualifications:- Master’s Degree in Social Work.- LGSW, LCSW or LICSW or a temporary Social Work permit and mustobtain permanent licensure within the first six months ofemployment OR LPC or provisional licensure certificate.
